Service Coordinator
Every participant in the HCS program is matched with a service coordinator, whose role is to:
Develop and oversee the execution of the Individual Service Plan (ISP), ensuring that personalized care is coordinated both within the HCS framework and through additional services.
Individual Socialization Skills Program
This program is structured to aid participants with daily social interactions and includes involvement in leisure activities. It aims to enhance their abilities in independence, self-sufficiency, social engagement, everyday life skills, and setting of personal achievement targets.
